Photography - I give permission to FKTennis to photograph my chid to appear in media online and printed now and in the future.

Rollover enrolment - I DO NOT want my childs enrolment to automatically roll over to the next term.

Emergency - I give permisson to FKTennis to call 000 and administer first aid in the event of an emergency.
